SupportBring a friend or family member to your appointments. They can take notes in a notebook or tape record the visit so that you can focus on the conversation during your visit and review the information afterward. You will receive a lot of information on your first visit; a second set of ears and hands will be helpful. Insurance and CopaymentBefore your first appointment, call your insurance company to find out if you need preauthorization for procedures or treatments. If you do, contact our preauthorization representative at 585-0100 before your appointment. Also ask your insurance company if you need a referral to see one of our clinic doctors. If you do, please contact your primary doctor and bring the referral letter or form to your appointment. Most insurance companies require that you make a copayment before each visit. If your insurance requires a copayment, please bring that amount with you to each appointment. We accept cash, checks, Visa, MasterCard, and American Express. Bring your insurance card and prescription card with you to the appointment. We will need the information printed on the cards to bill your insurances for the services provided. Obtaining Medical RecordsYou will need to bring your medical records to your appointment or have them sent to us by the doctor referring you. New privacy laws require that you sign a “Release of Information” form authorizing your doctor to send your records to us. Please sign this form at your doctor's office. You may be asked to bring radiology films or pathology slides as well. If you have seen any other doctors regarding this condition, please bring those records with you. List of MedicationsIt is important that we have a complete list of all medications (including vitamins and herbal remedies) that you currently take. Please complete the Medications and Chemotherapy List located in the Recording Your Care section using the information printed on your medication bottles. Registration and Check-inOur preregistration team may contact you by telephone prior to your visit. They will get information such as your address, insurance information, and an emergency contact person. Otherwise, we will get this information from you when you arrive. When you arrive on the day of your appointment, check in at the reception desk in the appropriate clinic. You will be told when you schedule your appointment to check in at Clinic A, Clinic C or Clinic E. (See Map of Clinic Locations.) There they will accept your copayment, provide you with information, and inform the physician's staff that you have arrived for your appointment.
